FRISCO ISD IS OFFERING FLU VACCINATIONS TO STUDENTS AND FAMILIES. ALL PARTICIPANTS MUST REGISTER ONLINE. PLEASE FOLLOW THE INSTRUCTIONS BELOW TO REGISTER FOR YOUR LOCATION AND TIME. MAKE SURE YOU HAVE YOUR CONSENT FORM ALREADY FILLED OUT AND PHOTOCOPIES OF YOUR INSURANCE CARD AND DRIVER'S LICENSE WITH YOU WHEN YOU ARRIVE AT THE CLINIC. WE CANNOT MAKE COPIES OF THIS INFORMATION FOR YOU. . SPACE IS LIMITED AT THESE CLINICS. IF THE SCHOOL YOU ARE WANTING TO REGISTER FOR DOES NOT SHOW UP ON THE DROP DOWN LIST THAT MEANS ALL OF THE SPOTS FOR THAT LOCATION HAVE ALREADY BEEN FILLED. PLEASE REGISTER FOR A DIFFERENT SCHOOL IN YOUR AREA. We are able to accept the following PPO type insurances: • ALL TRS Plans offered through Blue Cross Blue Shield- covered at 100% • Aetna* • Blue Cross Blue Shield* • Cigna * • Humana • United Health Care • Medicare Part B (Flu shots only-please provide a copy of any supplement plans also) • CHIP (age 18 and under) • Medicaid (age 18 and under) We do not accept any HMO plans, with the exception of BCBS TRS Primary and Primary+. *We are unable to accept Aetna & Blue Cross Gold, Silver & Bronze plans and Cigna Local Plus. Please see the consent forms for other known exceptions of plans that do not cover vaccines at 100%. A paper copy of your current insurance card and driver’s license are required. Please bring your paper copies with you when you receive your vaccines. Electronic cards cannot be accepted. If you do not have an accepted insurance the cost for the flu vaccine is $27 for adults and $10 for children. (Payable by cash or check only.) After you register you will see a confirmation page containing a link to a consent form. Please bring this, already filled out along with your photo copies. This will ensure the shortest possible wait. Registration times are requests only and you should still expect a short wait during the clinic. Arriving at the clinic with your photocopies and your consent form already filled out will help keep the wait times to a minimum. Depending on the carrier, insurance claims may be filed as Community Health and Immunization Services or Staying Healthy Medical Services. Thank you!

At the time of my appointment I will be made aware of certain risks that may be associated with the vaccine(s) including the potential for allergic reaction and I will be offered the opportunity to ask questions and I believe that the benefits outweigh the risks. I assume full responsibility for any reactions that may result. I authorize Passport Health to disclose my health information to my state immunization registry, my physician, my employer and my insurance company. I authorize Passport Health to notify me of future health events, services or products. I hereby acknowledge receipt of Passport Health's Notice of Privacy Practices. If I am signing this consent on behalf of another individual, I hereby certify that I have the legal right to do so as the person's parent or legal guardian, pursuant to a health care power of attorney, pursuant to a court order, or pursuant to some other legal right to consent to health care for the individual. I agree that the insurer or payer listed above is my primary medical coverage and that I am responsible for payment if services are not covered for any reason or are subject to any co-pays, deductibles, coinsurance or prior authorizations.
